The way that “push notifications” works, is when you set them up in APNS, (Apple Push Notifications Service) once you open the app and if it is registered to APNS. After that you’ll be asked, “Do you want the app to send you a notification?”

So you open the Application, and the app sends a message to APNS. It comes back with a token and at that point, you can enable push notifications on your iPhone. Push notifications are mostly used in chat or message applications or alarm services.
Let’s say you are chatting with a friend on an app that also has the app installed. You select his name, and allow push notification on iPhone, this will go to the APN server.
